The Haldimand Family Health Team is now housed in the new
"Primary Healthcare Centre", 334 Broad Street West,
Dunnville, Ontario, N1A 1T2. Please see the

Haldimand Family
Health Team now has the services of a full time Social Worker
Rebecca Stark-Lammel, B.A., B.S.W., RSW
located in the Primary Health Care Centre.
Services offered:
* Individual, family and couple therapy
* Community outreach and advocacy
* Short term counselling
* Social Rehabilitation and Life skills
training
* Assessment and short term treatment of
mental illness or substance abuse
* Psychosocial support to help
individuals deal with illnesses,
depression, anxiety, depression,
bereavement, caregiver stress, parenting
issues, anger management and past abuse
Patients who are rostered to the family
health team can speak to their doctor about a referral to this
service.
