This wonderful new service is one that was developed when HWMH was granted 4 temporary “Back To Home Beds” in of July 2009.
The primary goal of these beds is to provide an area where patients, who no longer require acute care services, receive the appropriate level of care. The intent of this care is to assist the patient to recover strength, endurance and functioning in order to enable him or her to “return and age at home”. Emphasis is placed on extra physiotherapy, reactivation through utilization of an activity program, regular participation in communal dining and other services identified by ongoing assessment of individualized needs. The length of stay is determined by the patient’s progress.
Eligibility of each potential candidate is determined by CCAC, while management of the beds is a collaborative effort between the hospital & CCAC.
